1. Traditional Medicine:

Traditional medicine is comprised of knowledge, skills and practises based on experience, observation of different cultures. It is not necessary that the action of the medicine has to be explicable. This medicine can be used for both mental and physical health. Traditional medicine is also known as alternative medicine where herbal treatment is the most popular form of treatment being used in 70% - 80% of the region. Traditional medicine includes ethnomedcine, ethnobotany and medical anthropology. Ethnomedicine is the study traditional medicine which is practised by different ethinic groups. Ethnobotany is the study of plants to discover their practical use through the knowledge of the local people. Richard Evans Schultes is referred to as the “father of ethanobotany”. Medical Anthropology studies the relation of heath and diseases, health care systems and biocultural adaptation and how well are they related to one another.Some of the famous and approved methods include acupuncture which can relieve several pain disorders, high blood pressure. Chinese herb Artemisia annua has been found to be resistive against resistant malaria. Another famous plant is Sutherlandia microphylla which may help to relieve HIV patients from its symptoms.

2. Traditional and Alternative Medicine:

Alternative Medicine or Psudomedicine are the practices or beliefs to cure any certain illness but its working principle cannot be explained or is impossible to prove with logical explanation. In some cases the risks of alternative medicine outweigh their benefits. Sometimes they are also ineffective to cure the illness and have toxic effect on your body. Some of the alternative treatments include acupuncture, acupressure, aromatherapy, balneotherapy, biofeedback, chiropractic, reflexology, reiki. Acupressure is like acupuncture but it involves hands instead of needles. It puts pressure at “energy point” which when blocked can create certain illness. Acupuncture has been quite famous with the western culture. It can be used to treat pain and depression. Aromatherapy is the use of essential oils such as those extracted from roots, leaves, seeds etc. This can be used to massage an effected part or can be taken orally to promote relaxation. Balneotherapy is the use of water to providing relief. The water can be used to soothe skin from acne and swelling and also relief the body from anxiety. Biofeedback allows people to control bodily processes that happen involuntarily. This includes heart rate, muscle tension, skin temperature, blood pressure, etc. Chiropractic is mainly focuses on musculoskeletal and nervous system. They mainly focus on “hypomobile” joints to restore them to their normal functioning. The most common procedure performed by them is “spinal manipulation”.

3. Ayurveda Medicine:

Ayurveda itself can treat many issues in the body such as stress, anxiety, blood pressure and cholesterol, recovery from injuries, promotes anti-oxidant rich diet, maintains weight, lowers inflammation and keeps the hormonal level in balance. It is a 5000 year system of treating ailments which has its roots of development in India. The recent popularization of alternative and complementary medicine in the west has proved to have a positive effect at the growth of Ayurveda medicine. The word Ayurveda in Sanskrit means “lifespan build on knowledge.” Ayurveda works by maintaining a balance by having a healthy diet and lifestyle changes. Unlike western medicine, Ayurveda medicine takes a person’s bioindivitualilty into account along with entire body-mind-spirit connection. Thus rather than treating the symptoms of the disease, Ayurveda relies on the power of the mind over the body to treat the disease from its core.

4. Homeopathy Medicine:

It is the treatment of body’s ailment by using substances at highly diluted form which might otherwise cause harm to your body. The principles of Homeopathy are sometimes not possible to be explained by modern science. It works by eliciting strong response from the immune system by adding adjuvants in a highly diluted form. Homeopathy unlike western medicine takes into account not only the symptoms one is suffering but also the mental state, lifestyle and diet before prescribing any treatment. The side effects from Homeopathy is generally nil to very negligible. Homeopathy contains three principles. The first being, “like cures like”, the second being, “The minimum dose” and the third being “The single Remedy”. Some of the very famous homeopathy medicines are Arnica, Allium cepa, Chamomilia, Ignatiam etc. Homeopathy like Ayurveda can be used for treating several kinds of diseases including, depression, asthma, hay fever, ear infection, arthritis, high BP, etc.

5. Acupuncture:

It is derived from ancient Chinese medicine. It works by stimulating sensory nerves under the skin and the muscles and release natural substances known as endorphins. A course of acupuncture provides more pain relief than a single treatment does. It works on the principle that the body can get recover from ailments by restoring the flow of Qi in the body. It involves insertion of needles which are a few centimetres long. They are for a single use only which are pre-sterilised. A maximum of 12 points can be used for every session and sometimes even more depending upon the symptoms. The acupuncturist may decide to just put it skin deep, or deeper. The needles can be kept for a few minutes to up to 30 minutes. It is generally safe to undergo acupuncture if you are pregnant.

6. Herbal Medicine:

It is the study of different types of plants and uses it for medicinal purpose or uses it as a supplement to food.  Many modern medicines have plant derived substances. Herbal medicine is also known as phytotheraphy and also sometimes include fungal and bee. Nearly 25% of the drugs worldwide are created from plant. Earlier plants could be used to treat the illness easily but the overuse of drugs for treating the disease rather than the root cause led to production of resistant microbes. Now millions of dollars are being invested to find out more plant based herbs. Herbal Medicine has proved that food is medicine. Herbal Medicine includes traditional medicine of different regions and also Ayurveda medicine. It has many benefits over conventional medicine like it’s cheaper, easier to obtain, has healing properties.

7. Herbal Cosmetics and Nutraceuticals:

Herbal Cosmetics are derived from plants and generally do not have any negative side effect on the body. Just like any kind of cosmetic, herbal cosmetics are used for body’s cleansing, beautifying or increasing attractiveness. They also do not provoke any kind of allergies. Since they are made from natural products, they can get easily incorporated with the skin and hair. On the negative side, they take more time to work than allopathic cosmetics. They are also difficult to produce since their down streaming process is difficult and costly. Nutraceutical are dietary supplements and food additives that do not come under regulatory category. They have physiological benefits which protects against chronic diseases. The diseases include, improve health, and delay aging. The term Nutraceutical combines two words, Nutrient” and “pharmaceutical”. The main focus of this is to prevent the beginning of any disease. They are supplements which can be taken in form of pills, capsules, liquid or even powder. It has many health benefits include, antioxidant defences, cell proliferation, mitochondrial integrity and gene expression.

8. Holistic Medicine:

Holistic medicine follows the principle of “Prevention is better than cure”.  Such physicians emphasize more on the healing power of love, hope, humour to release the toxic consequences of hostility, shame and greed. They believe that unconditional love is the most powerful medicine. The physicians tend to develop unconditional love for their patients. It is the science that addresses the whole body including mind and spirit. It falls under the category of alternative medicine practise. It works by restoring the unlimited free flow of life force through body, mind and spirit thus uniting them. The treatment procedure also takes into account the analysis of physical, nutritional, environmental, and emotional and lifestyle elements. They take into account the education of lifestyle and self-care. They search for underlying causes of the disease instead of treating the symptoms only.

9. Medicinal Plants:

Medicinal plants are used under traditional medicine. They have been shown to have defensive actions against insects, fungi and herbivorous mammals. A whole plant contains a diverse phytochemical. Thus using the whole plant for a certain phytochemical only reduces the effect of that certain phytochemical. Moreover, the efficacy of any phytochemical content can be approved for public use after rigorous lab tests. Some of the most famous Medicinal plants include ginger, lavender, garlic, spinach, catnip, thyme, tea tree, lady ferns, sage, peppermint, marigold, cayenne pepper, Echinacea, dandelion and chamomile etc. Ginger is good for digestion and allergies. Lavender oil keeps dandruffs away. Garlic is good for boosting your immunity and fight against allergies as well. Spinach has antioxidant properties which protects DNA. Catnip is good against headache and migraine. Thyme is good for prevention against food borne disease. Tea tree is used against infection of nail, skin, and mouth and head lice. ADMET and drug development requires ethnobotany to search for pharmacologically active substance in nature.

10.  Naturopathic Medicine:

As a naturopathic physician, they educate their patients to take responsibility for their health. They not only take the physical health into account but also the mental health of that person. Thus treating the person as a whole. Naturopathy is based on natural healing and treating the root cause of the disease rather than just treating the symptoms. It is a distinct medical practise which emphasizes on self-healing and prevention rather than treating the symptoms. The principles of naturopath medicine include using of healing power of nature, identify the root cause of the disease and treat it, using the least harmful method so that it has the least amount of health hazard and side effects.

Unlike the Christians who viewed disease as a punishment of God, the Islam looked at it just like any other problem of mankind to be solved. Though westerners credit William Harvey for discovering the blood circulation, the Islam’s discovered it 300 years before its discovery though the information was incomplete. They also discovered that some diseases like smallpox and tuberculosis were infectious and the quarantine process to limit its spread. An Arabic doctor named Al Zahrawi wrote a 30- volume book of medicine and surgery and invented almost 200 surgical instruments which are even used today. The Islamic physicians only used such herbs which showed to have a positive effect on the body. Arab government ordered its pharmacy to ensure the purity and overall quality of the product sold. These pharmacies started to spread through the Muslim world and beyond. It is a form of Traditional Medicine practiced in Mughal India and in Muslim culture in South Asia.  It is also influenced by Chinese Traditional Medicine. The diagnosis by this kind of medicine requires a thorough knowledge about the signs, symptoms and temperament. The balance of the natural functioning in the body is maintained by Quwwat-e-Mudabbira-e-Badan. Any derangement in this leads to imbalance of humours or akhlat of the body. This change in humour in an abnormal way leads to pathological or physical problems in the body. Depending upon these three principles of imbalance and balance the treatment is suggested. Arabic Medicine is also known as Islamic Medicine.

12. Chinese Medicine:

Chinese Medicine evolved in ancient China over a thousand years ago. It includes many methods of treatment like Tai chi, acupuncture etc. Unlike normal drugs which have to prove their efficacy to the FDA, these are used as food supplements and don’t have to prove what they claim. Chinese Medicine sees the whole body as a miniature version of the universe. It believes that there is a balance between complementary forces named Yin and Yang. Imbalance in these forces leads to occurrence of diseases. Qi is a vital energy which flows through the whole body and provides energy for proper functioning. Chinese herbal medicine consists of several medicinal substances, especially plants, minerals and some animal products. Acupuncture is the stimulation of certain points in the body which are considered to be vital for the flow of life energy. This helps to restore these vital points and cure a disease. Tai-chi involves gentle dance like movements coupled by breathing, relaxation and mental focus.

13. Natural Products:

It is a compound or chemical substance which is produced naturally from living organisms. Natural products are used not only as dietary supplements but also as cosmetics. They contain enormous amount of structural and chemical diversity. They are richest source of molecular scaffold. However, the action of natural products in a particular situation can never be predicted. They still remain the best source of drug and drug leads. Since the year 1940, 131 molecules which are discovered as anticancer activity, 85 of them are either natural products or naturally derived.

14. Drugs from natural sources:

 Drugs from natural sources are usually crude drugs which are derived from biological or mineral sources. Such drugs do not require additional processing as they already contain pharmacologically active ingredients. The drugs include ginseng, dry root of rhubarb, dried milk juice of opium and whole dried insect like Spanish fly. Drugs obtained from animal sources are usually whole animals or parts of the animal like thyroid organ, liver, non-peptide toxins, etc. Other sources include that of microbes which provides many lifesaving drugs such as penicillin. Plants have been a source of many novel drugs. Nature has always been a major source of new structural leads and providing new structural leads. Many important drugs such as antibiotics like penicillin have been developed from microbes. The genetic knowledge leads to novel screens and genetic techniques which also allows genome mining. Due to new genetic tools which has permitted isolation and expression of earlier unidentifiable genes has led to discovery of new products.

15. Pharmacognosy and Traditional Medicine:

Almost 80% of the world population rely on traditional medicine for treatment. In traditional medicine the active ingredients are isolated from plant extracts. Sometimes it is used to mimic a natural plant compound. Thus the natural molecules act as good models for mimicking the drug molecules. Taxol is a good example of anticancer drug which is obtained from baccatin III. It is isolated from taxusbaccata. Pharmacognosy deals with the effectiveness of particular medicinal products such as in traditional medicine like homeopathy medicine and acupuncture etc. It is the study of chemical, biochemical and biological properties of drugs or potential drug subjects. It is also known as the study of crude drugs.  The term Pharmacognosy was first coined by Seydler, a German botanist.  Medicine has been derived from plants since eternity by mankind to cure diseases. It has always been a go-to for finding new drugs.

16. Toxicology Studies of plant products:

 Medicinal plants are mostly sold as dietary supplements rather than medicines. Due to this reason they have less stringent rules by the FDA than actual drugs in the western world. Toxicology studies are important to denote the efficacy of the doses of the medicine and what number doses are safe until it becomes toxic. It is the study of chemical interaction with the living system and how it affects its normal processes. Toxicologists know that nothing is risk free after a certain point. Their studies help us save from overdose. The best and common example is oxygen. When it is used in limited condition, they are safe to use but its overdose is toxic. Toxicology helps us find out the evidence as to what their mode of action is. The meticulous toxicology studies provide a guideline about the safe use of the plant product. Thus toxicology needs to list the proper side effects of a medicinal plant before they can be marketed.

Homeopathy is the use of highly diluted solutions which in concentrated form may be harmful to the body. Thus it works just like a vaccine does. Naturopathy is the use of nature’s healing power to treat ailments. Reflexology is the application of pressure at specific points on the feet and palm which are believed to benefit certain parts of the body. This helps to improve the overall health of an individual. Reiki is the use of life energy to heal and is based on the idea of “life force energy”. Thus these were some of the method used in alternative medicine. Alternative medicine methods are usually diverse and it is difficult to prove their efficacy hence is not accepted as standardized methods in the western world. Some of the well-known alternative methods include acupuncture, aromatherapy, and Ayurvedic medicine, Balneotherapy, Biofeedback, Chiropractic, Homeopathy, Naturopathy, Reflexology and Reiki.  Acupuncture is the use of needles to unblock the Qi points (Life energy points) which when blocked causes certain diseases. Aromatherapy is the use of essential oils to promote healing. Ayurvedic medicine includes use of herbs and massage, specialized diets with the intent to balance the body and mind to treat any illness. Balneotherapy is the use of water for treating illness. Water is known to rejuvenate skin. It can also be used to boost your immune system. Biofeedback is the method to control involuntary processes of the body such as heart-rate, blood pressure, muscle tension etc. The therapists teach the patients methods of relaxation to help them control these problems. Chiropractic is the treatment of musculoskeletal and nervous system disorder by resetting their alignment.

INTRODUCTION

India and Africa use 70 % and 90 % depend on traditional medicine to meet health care needs. China uses 40 % of the time traditional medicine to cure ailments.  WHO has observed an exponential growth in the use of traditional medicine worldwide in these few decades. In the US about 38% of the adults and 12% of the children are using some form of traditional medicine.

During the 2003’s, expenditure associated with “alternative” therapy in the United States was estimated to be US$13.7 billion. The growth of herbal medicine had doubled till the year 2005, growing faster than any other alternative therapy. The total amount of expenditure per year spend on traditional medicine by Australia, Canada and the United Kingdom is estimated to be US$80 million, US$1 billion, and US$2.3 billion, respectively

The commercial value of the ethnobotanicals market is huge. In the 2005, the total turnover of non-prescription-bound herbal medicines in pharmacies was equal to almost 30% of the total turnover of non-prescription-bound medicines in Germany, and in the United States, the annual retail sales of herbal products was estimated to be US$5.1 billion. The medicinal plants used by the Indian herbal industry consist of 960 species out of which 178 are produced in high volume. The annual value of herbal medicine manufactured in China had reached 17.6 billion Chinese yuan by the year of 2007. This logarithmic growth has continued, and annual revenues in Western Europe reached US$5 billion in 2008-2009. Sales of herbal products totalled US$14 billion in 2010 in China, and revenue from herbal medicines in Brazil was US$160 million in 2017

FUTURE OF TRADITIONAL & NATURAL MEDICINE

Both traditional & natural medicine has seen a rapid growth in the industry in the past few decades and the graph has stayed logarithmic and it intends to stay so. US has become the largest importer of Traditional medicine in the past few years which is now challenging the international import markets at Hong Kong. By the year of 2019, the absolute $ opportunity is supposed to be around $8800Bn with an increase by $140Bn between 2019 and 2028. The market of traditional medicine has been over $130Bn till date. The trend to use traditional and natural medicine has gained popularity in the western world as supplements or nutraceuticals with health claims which are already been approved by FDA. The reason they have gained popularity so much in the western world is because they have less regulatory rules compared to pharmaceuticals drugs. Other traditional medicine includes homeopathy medicine which has gained popularity due its effectiveness and low side effects.

Ayurvedic medicines have seen the most exponential growth in the field of traditional medicine. It is valued at more than $75Bn in 2019 and has an annual growth rate of 6.6%. The herbal medicine market is valued at $71.19Bn and expected to grow fetching profitable gains. Medicinal plants are also gaining popularity due to their comparatively lower costs to allopathy. It has also been used to treat SARS severe acute respiratory syndrome in China. Tannins, flavonoids possess anti-cancer properties. They are expected to gain popularity due to the increasing cases of cancer.
